- The number of individual treatment sessions requested.
- For GP referrals, confirmation that a Mental Health Treatment Plan has been prepared.
- Referring practitioner’s name, provider number and contact details.

Number of Sessions
Initial course
A maximum of six individual sessions.
Further treatment
A new referral is needed for each subsequent course.
Each course can include up to six sessions, subject
to the patient’s remaining annual entitlement.
Annual entitlement
Eligible patients can claim Medicare rebates for up
to 10 individual sessions per calendar year, including
sessions received from other providers.
For example, after six sessions in the same calendar year, a further referral may cover the remaining four sessions, if clinically appropriate.
A Mental Health Treatment Plan alone is not a referral. Please send the referral letter and, where available, a copy of the plan before the patient’s first appointment.
